Service Coordination

Primary Care Partnerships were established networks of local health and human service organisations working together to find smarter ways of making the health system work better, so that the health of their communities is improved.

HealthWest Partnership was one of 30 Primary Care Partnerships (PCPs) in Victoria and its catchment included Melbourne’s western areas of Brimbank, Hobsons Bay, Maribyrnong, Melton and Wyndham. HealthWest brought together 32 health agencies, community organisations and local government members in a voluntary alliance to strengthen health and community services’ planning and collaboration in Melbourne’s west.
